March In-person Support Group: Advance Directives

Our Thursday, March 21st HD support group will be in-person.  Dinner will be provided. 

We will be completing our Advance Directives together, including our Medical Power of Attorney form and Mental Health Power of Attorney form. The forms will be provided to you.  Dawn will have copies of the Living Will form, but it is unlikely we will have time to get that form completed. You can take it home with you. 

Anyone in attendance that evening can go home with notarized Mental Health and Medical POA forms.  We will have several notaries joining us that night to provide assistance.

Our meeting information is below:

When:  Thursday, March 21st
Time:    6-8pm
Where:  HDSA Center of Exellence at Barrow Neurological Institute 
              240 West Thomas Rd, 3rd floor - Meeting Room
Parking: 3rd Avenue Garage Across from the HD COE- bring me your parking ticket for validation
Food:     Dinner provided.  Please bring your own drinks

***  Please Note,  the front doors to the 240 building are locked at 5pm.  I will have a volunteer at the doors from 5:45pm to 6:15pm. The volunteer will also key you up to the 3rd floor, as the elevators are also not operating in the evening.  If you come before 5:45 or after 6:15pm, you will not be able to get to the meeting room.  Please be sure to arrive during this time frame.  

***  Also note, in order to have your documents notarized, you will need to have either a driver's license, photo ID, or Passport ID.
